2. Punan ang lahat ng kailangan sa form
Ang inyong MID # ay maari niyong makita sa binigay na resibo ni cashier sa pagbayad niyo ng inyong bill.
3. Punan lahat ng may * at i click ang SUBMIT
4. Once na na click niyo na ang SUBMIT ay automatic na may record na kayo for registration.
Ganun lamang kasimple ang pag fill up ng registration google form sa AGMA.
